We are seeking a qualified
PRN Hospice Chaplain for our Hospice office location. This office is located in Newport News, VA and provides service to the cities of Southern Virginia to include Peninsula cities of Newport News, Hampton, Yorktown, Poquoson, Williamsburg, Gloucester and southside cities of Va Beach, Norfolk, Portsmouth, Chesapeake, Smithfield and Suffolk.
JOB SUMMARY: AS a member of the interdisciplinary team, the
Hospice Chaplain provides spiritual support to the patients and families in keeping with the patient’s/family’s desire and religious beliefs. The
Hospice Chaplain will understand and have knowledge of the hospice philosophy of care, spiritual needs and related to end of life care, loss and bereavement.
REQUIRED QUALIFICATIONS: - Graduate of accredited seminary school of theology, or appropriate certification in hospital or pastoral ministry
- Experience in working with patients and families dealing with life threatening illness and death
- Committed to nurturing the spiritual component in the care of the terminally ill
- Experience in a health care setting preferred
- ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
- Coordinates and implements all activities relating to the spiritual aspect of the hospice patient care
- Provide direct spiritual support to patient/family
- Work with staff, local clergy and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss
- Provide bereavement services
- Assist with Personal Touch Hospice memorial service
- Assist with volunteer program
- Conduct home visits and maintain proper records for visits to patients/families
- Assist with funeral planning/conduct memorial services if requested
- Arrange for on-call availability for chaplain services
- Attend interdisciplinary group meetings, assists with the development of the plan of care in relation to patient’s/families spiritual needs
